Understanding insurance coverage for infertility treatment & IVF costs

Insurance coverage for infertility, artificial insemination and in vitro fertilization (IVF), one of the most popular and well-known fertility treatment options, varies greatly. Most insurance plans cover no infertility services, a few may pay for all of it, and others are somewhere in between.

Louisiana is not one of the 15 states where laws mandate insurance coverage of assisted reproductive technologies such as IVF. This means that a person’s insurance plan will dictate fertility coverage, if any, so it’s important to check with your insurance provider before beginning treatment.

Each private healthcare insurance plan can be different, particularly regarding coverage for infertility treatments. Knowing plan benefits, exclusions and all other factors is the responsibility of our patients.

We know it can be hard to predict what fertility treatment expenses will be and what information you will need from your insurance provider. To better understand your insurance benefits and potential costs for fertility treatment, we’ve compiled this checklist.

  • Connect with your insurance carrier to find out whether your policy requires a referral or preauthorization for coverage.
  • Review your policy to determine specific insurance coverage (if any) for fertility treatment.
  • If benefits are included, look into limitations, exclusions or caps to benefits (yearly maximum benefits or specifics on when IVF benefits kick in, for example).
  • Plan to pay for services not covered by insurance (co-payments, deductibles, out of scope services, etc.). These are considered “self-pay” fees and are due at the time of service unless a treatment deposit or financing plan has been set up in advance.
