When it comes to health, questions often don’t begin and end within each business day. So we believe answers should be available when you need them with The Fertility Institute’s secure patient portal.
eIVF Patient Portal
Here patients can easily communicate with our team, access patient records and forms, view lab results, schedule appointments and appointment reminders, view account balances, and pay bills online.
Our secure eIVF patient portal helps you stay connected and informed.
With your portal, you can:
- Communicate securely with your care team
- Find your medical history
- View test results and upcoming appointments
- Manage medications and instructions
- Access and sign important documents and forms
- And more

For New Patients
Prior to your first appointment, our team will provide a temporary login and password to access the eIVF Patient Portal. If at any time you have trouble setting up a new account, please contact our office.
Important Note: Please be sure to complete the information for both partners via a separate “log in” with a different user ID and password for each partner.
Step 1: Getting Started
- Prior to your first appointment, our team will provide a temporary login and password to access the eIVF Patient Portal.
- You must complete your first login via a desktop computer. After logging in for the first time on a desktop computer, you can generate an app code to be able to log in via the mobile app for future logins.
- Click the “eIVF Patient Portal” button to go to the patient portal landing page.
- Click on the “sign in button” on the top right-hand of your screen
- Once the login appears, enter the username and password you were given and select login.
- After logging in for the first time, you will be prompted to update your password.
Step 2: Completing Consents
- This information needs to be completed before your initial consultation with one of our physicians.
- After creating your new password, you will be prompted to fill out your patient demographic information, medical history, and consent forms.
*Note: Demographic changes will not appear immediately.
- Once demographic information is completed, click on the History tab and click on “Patient Evaluation” to complete your medical history.
*Please complete this at least 72 hours before your appointment
- Click on “Profile” and Upload insurance information in the Profile section
- Complete and sign all Financial and New Patient Consent forms
- When finished click “log off”
- Repeat the process for your partner using their separate login
Step 3: Download the mobile app
- After logging in on the desktop version, you will see a box that says “get app code here”. This is located on the home screen and has an Apple and Android symbol on it. The code that you receive is the one-time registration code you will need for the App login. This code is NOT the same as your username and password.
- You will then need to download the eIVF CareSync app on your mobile device. This can be done through the Google Play or Apple App Store. You can also find the link for both versions below.
- Once you have successfully downloaded the app, you will be asked to input a registration code, this is where you will enter the one-time app code that you generated on your desktop.
Notice for Current Patients: Portal Messaging & Availability
Portal messages are reviewed during regular office hours (8 a.m. Monday – 4 p.m. Friday). Please avoid messaging the practice on weekends and holidays, as responses are delayed. For immediate or urgent assistance, please contact the on-call physician.
Online Payments in PersonaPay
Paying for your care should be simple, secure, and stress-free.
Through our PersonaPay portal, you can:
- Make a payment online
- View billing statements
- PersonaPay is very intuitive and guides you through the process of making a partial payment or payment in full.
How can a patient make a payment?
Patients can make a payment in the following ways:
Self Service
- via a QR code printed on their paper statement
- via a link from the patient’s e-statement
- via PersonaPay portal
