What Is Controlled Ovarian Hyperstimulation?

COH is a fertility treatment that helps ovaries develop and release several eggs during ovulation instead of just one. The increase in eggs raises the likelihood of conception because there are more eggs for sperm to potentially fertilize.

COH is commonly used in conjunction with assisted reproduction technologies such as intrauterine insemination (IUI). It is also used to develop multiple eggs during an in vitro fertilization (IVF) cycle. During an IVF cycle, the eggs are retrieved and fertilized in our state-of-the-art IVF lab before one embryo is selected and inserted into the uterus. Extra eggs or embryos can be frozen and stored for future use.

COH is typically attempted for three to six cycles. If this method doesn’t work, your fertility specialist may recommend IVF or another personalized fertility treatment method.

Who Might Benefit from Controlled Ovarian Hyperstimulation?

Your fertility specialist will help you determine if COH is right for you. We will create a personalized fertility treatment based on your medical history, your partner’s history, and fertility assessment.

You may be a good candidate for COH if:

  • If you are a woman under 41 with unexplained infertility.
  • Your partner’s sperm concentration and motility are good/high.
  • You have endometriosis, PCOS, or another condition that may affect your ovaries or ovulation.

COH will not cure the infertility you may be experiencing, but it will treat it temporarily to increase the chances of conception. If you wish to grow your family again in the future, you may need to consider fertility treatments, like another cycle of COH or IVF.

Who Might Benefit from Controlled Ovarian Hyperstimulation?

You and your fertility specialist will have a consultation before you begin COH. Your fertility specialist will walk you through the process, so you know what to expect.

On days 2-4 of your menstrual cycle, you will have a blood test and ultrasound, so your fertility specialist has a baseline to measure your progress. Then, you’ll receive medication to stimulate your ovaries. Throughout the cycle, we will monitor your progress, including the size and development of your ovarian follicles. Once your ovarian follicles are mature, you’ll receive a “trigger shot,” which will prepare your ovaries to release the eggs.

About 36 hours after your “trigger shot,” you will have sex or undergo intrauterine insemination to hopefully conceive. IUI is an in-office procedure where your fertility specialist inserts a catheter into the uterus and injects the concentrated sperm so the sperm can more easily access the egg. You can return to normal activities on the same day. Many patients describe IUI as similar to getting a pap smear.

Your fertility specialist will give you a blood test two weeks later to determine if you are pregnant.
